As a Home Care Registered Nurse, you will play a crucial role in providing and directing nursing care to patients in the comfort of their homes, following physician prescriptions and adhering to all applicable laws and agency policies. You'll collaborate with a team of healthcare professionals to ensure seamless patient care, maintaining clear communication with physicians, agency staff, patients, families, and community resources.

Key Responsibilities:

• Deliver high-quality clinical services while adhering to infection control standards.
• Coordinate patient care with other team members from admission to discharge.
• Conduct clinical nursing assessments according to federal/state requirements and payer guidelines.
• Verify patient eligibility and medical necessity for services based on payer and agency policies.
• Develop and update individualized plans of care in collaboration with community providers.
• Adjust frequency and duration of care plans as necessary with physician approval.

Qualifications Required:

• Valid and unrestricted RN licensure in your state.
• Current driver's license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
• Ability to provide care in diverse home environments without regard to age, race, creed, color, sex, disability, or economic status.

Preferred Qualifications:

• At least 1 year of RN experience.
• Ability to work independently.
• Strong communication, writing, and organizational skills.

Compensation is determined by several factors, including local labor markets, education, experience, and certifications. In addition to your salary, a comprehensive benefits package is available. Benefit offerings include incentive and recognition programs, equity stock purchase, and 401k contributions (subject to eligibility requirements). The hourly pay for this position ranges from $28.27 to $50.48 for full-time employment. Compliance with all applicable minimum wage laws is ensured.
