Express Healthcare Staffing of Tualatin Oregon is seeking a travel nurse RN Long Term Care (LTC) Case Management for a travel nursing job in Seattle, Washington.

Pay Breakdown:
• Hourly: $30/hr Taxed ($1,200/wk Taxed) +
• Per Diem Eligibility: $1,750/wk =
• $2,950/wk Gross Eligibility

REQUIRED: BLS, WA or Compact License

Guaranteed Hours: 40hr/week Contract
Years of Experience Required: 2 Years
First Time Travelers Accepted: Yes
Locals Accepted: Yes!

The RNCM establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination and monitoring, and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities includes: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from acute care centers and skilled nursing facilities. Additionally, this position will work closely with the IDT Social Worker and Physician to collect necessary medical and social information to ensure cohesive decisions are made which ensure the safety and well-being of participants.

**This position is primarily on site, but there are times when RNCM will need to drive to a participants residence for an assessment if needed. Mileage will be reimbursed for this time.**
