JOB SUMMARY: Obtains accurate, comprehensive medication history of patients. Interviews patient to obtain complete medication history. Communicates with outside sources to determine all current and active medications prescribed to the patient. Evaluates medication adherence based on patient interview. Accurately documents current medication history for physician reconciliation.
REQUIREMENTS: Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field. 3-5 years of experience. PTCB Certified. Maryland Board of Pharmacy Technician Registration.
