PRN RN – Hospice Nurse (Livingston, Cleveland, Conroe, and Surrounding Areas).

What You’ll Do:

• Provide skilled, compassionate nursing care to patients and families.
• Complete routine visits, recerts, and IDT notes, with opportunities to assist with admissions and on-call shifts when available.
• Collaborate with the hospice team and actively participate in interdisciplinary team (IDT) meetings every other week.
• Maintain accurate, same-day charting with a provided tablet.
• Travel within Cleveland, Livingston, and Conroe.

What We’re Looking For:

• Current RN license in Texas.
• Hospice or home health experience preferred.
• Strong communication and organizational skills.
• Reliable transportation and comfort with travel across the service area.
• Most importantly: a passion for providing meaningful care at the end of life.
