Your Responsibilities:

• Conduct comprehensive assessments to develop personalized care plans.
• Facilitate effective communication between patients and the interdisciplinary care team.
• Establish and maintain therapeutic relationships with patients through education and promoting self-advocacy.
• Utilize state-of-the-art technology for accurate documentation of skilled services provided.

Qualifications:

• Valid CT RN license and a registered nurse qualification.
• At least one year of RN experience; prior home care experience is preferred.
• Valid driver's license and reliable transportation are necessary.
• Proficiency in computer use is required.
